Enroll in Music Lessons!

The form below is to be completed only after you have already contacted us via phone, email, or contact form. We want to make sure we answer any questions you may have before enrolling.

Upon filling this form and paying the invoice we will send you, the student will officially become a member of Miami HofCA and reserve a spot for the academic year.

If you would like to try a lesson before enrolling, please fill out our tryout music lesson form instead.
